1. The psychologist conducting the therapy (“Specialist”), i.e. Monika Kusior, is obliged to maintain the confidentiality of information about the Client obtained in connection with the performance of the profession based on Article 14 of the Act of June 8, 2001, on the profession of psychologist and the self-government of psychologists (Journal of Laws 2001 No. 73, item 763). This obligation is not time-limited. It does not apply only in cases where the life or health of the Client or other persons is seriously endangered or where the law provides otherwise.

Since during the treatment you will be sharing with the Specialist information of a particularly private nature, please express your explicit consent to the processing of your personal data. Failure to provide consent will prevent the proper conduct of treatment. The consent given can be withdrawn at any time, however, the withdrawal of consent will not affect the lawfulness of processing that occurred before its withdrawal. Providing data during visits is voluntary, however, without access to information about the Client, the Specialist will not be able to conduct treatment properly.

2. The administrator of the data provided by you is Monika Kusior (NIP 6482743322/ REGON 361894578).

The data that is processed includes: name and surname, phone number or email address – provided during the registration of the visit, personal data provided by contractors and persons representing contractors, i.e. data contained in the contract, contact persons' data, and other data necessary for the execution of the contract, personal data provided during registration on the sitewww.psychoterapiakusior.pl or by phone or email, data concerning your mental health and other data obtained by the specialist during the ongoing treatment, data provided for the purpose of issuing an invoice.

Your data is processed for the purposes related to the provision of psychological, psychotherapeutic, and therapeutic services, for billing purposes, bookkeeping, maintaining a calendar and appointment reservations, issuing certificates, contacting another specialist, communicating between sessions, as well as keeping and storing any documents from the therapy in paper, electronic, and telecommunication form.

3. The personal data provided by you will be stored from the moment of booking the visit, for the duration of the treatment and for 5 years after its completion. 4. You have the right to access your personal data and update it, as well as to file a complaint with the supervisory authority.
